Who Qualifies?

Home health services are available to patients who meet specific criteria. Our team will work with you and your physician to determine eligibility and begin the referral process promptly.

Under Physician Care
The patient must be under the care of a physician, dentist, or podiatrist who orders and oversees the home health services.

Homebound Status
Patient must be homebound and in need of intermittent care โ€” leaving home requires considerable effort due to medical condition.

Face-to-Face Assessment
For Medicare/Medicaid, a current face-to-face assessment by your physician documenting the need for skilled care is required.

Skilled Care Need
Care must include โ€” but is not limited to โ€” assessment, observation, direct care, education, or managed care services.

Who Pays for Care?

Most major insurance plans cover home health services. Our staff will contact your insurance carrier to determine your eligibility and coverage prior to starting services.
